(1) The information required under Rules 14.8.304 and 14.8.305 shall be reported on forms designated by the department.
(2) The department shall provide ninety days notice prior to specifying or modifying any form.
(3) Subject to the department's prior approval of the report format, the department shall accept copies of reports filed with other federal and state agencies or private associations, if such reports provide all of the information described in Rules 14.8.304 and 14.8.305.
(4) Certified computer printouts, which are in the format of the reporting schedules designated by the department, may be submitted in lieu of the specified forms.
