(1) If, after service of the report of violation form the parolee wishes a preliminary on-site hearing, the parole officer shall notify the parolee of the time and place of the hearing by a notice of preliminary hearing form. This notice shall apprise the parolee of the same rights and privi-leges as were stated in the statement of charges. (Form A.)
(2) The notice of preliminary hearing shall inform the parolee that he/she has 48 hours to inform the parole office of his/her desire to have witnesses appear at his hearing. The parolee shall be given a request for witnesses form for this purpose. (Form B.)
(3) If the parolee is being detained pending hearing, the parole officer shall contact the requested witnesses and inform of the time and place of the hearing.
