37.107.125    REPLACING LOST OR STOLEN REGISTRY IDENTIFICATION CARDS

(1) If a registry identification card has been lost or stolen, the department must receive notification from registrant prior to replacing the registry identification card.

(2) Upon receipt of notification of a lost or stolen card by the registrant, the department will reissue as duplicate the registry identification card and send it to the registrant.
