(1) Each application for an initial license as a nutritionist under the act must be accompanied by:

(a) a completed application form;

(b) the initial license fee;

(c) a copy of the registration by the commission; and

(d) applicant's current original unopened National Practitioner Data Bank (NPDB) self-query report.
