(1) After issuance of a decision, a dissatisfied party requesting mediation shall file a written request for mediation with the ICCU within 15 days of the mailing of the ICCU's decision. The request for mediation is effective upon receipt by the department, not upon mailing. It is the obligation of the requesting party to ensure timely receipt of the request for mediation by the ICCU.
(2) A request for mediation may be filed in the following ways:
(a) by mail to P.O. Box 8011; Helena, MT 59604;
(b) by electronic mail to [email protected]; or
(c) by facsimile to (406) 444-4140.
(3) Upon receiving a request for mediation, the ICCU shall provide the mediator a copy of its decision and any other documents it deems relevant.
