(1) Each application for an initial license as a nutritionist under the act must be accompanied by:

(a) a completed application form;

(b) the initial license fee; and

(c) a copy of the registration by the commission.

(2) The board or its designee will obtain a query from the National Practitioner Data Bank for each applicant.
